We are happy to register new NHS patients within our practice boundary area (W1).
How to Register
To be registered at the practice, you will need to complete the GMS1 form and either the Adult Registration Form or the under 16’s Child Registration Form and return them to the surgery with your passport or ID card and proof of address.
Please complete the forms as incomplete forms will delay your registration.

Registration forms can be printed via the links below or collected from the surgery.
To register with our practice, you need to come personally to the surgery with a proof of address and one form of identification.
Acceptable identification includes:
- Driving License
- Birth Certificate
- HC2 Certificate
- Rough sleepers’ identity badge
- Hostel Registration/mail forwarding letter.
Acceptable proof of address includes (within 2 months prior registration):
- Property Tax Receipt.
- Utility Bill.
- Lease Agreement.
- Insurance Card.
- Voter Registration Card.
- College Enrollment Papers.
